JoslinCare team members will work with you to create an individualized pathway of care based on your health profile and lifestyle. This involves a program of education and multidisciplinary care to aggressively manage your diabetes. This may also include early referrals to specialists to prevent or delay the progression of complications.
Whether you are newly diagnosed with diabetes, want to improve your control or have realized that your efforts to manage your diabetes are not meeting with success, JoslinCare is a pathway you can follow.
Who should be a patient at Joslin?
This is one of the questions we are asked most frequently.
Modern diabetes care originated at Joslin. We're still the only institution focused exclusively on improving the lives of people with all forms of diabetes and its complications.
If you or someone you care for has diabetes, you know what a terrible impact this disease can have. However, we also know that if diabetes is managed properly, complications are not inevitable. We encourage everyone who has diabetes to ask for a referral to Joslin Clinic.
Our patients range from children to seniors—from people who have been newly diagnosed to folks who have received Joslin Medals for living with diabetes for 50 years.
They include patients with serious health challenges—as well as patients who are in excellent control and are working to maintain their health. The average A1c of Joslin patients is 7.6%, more than 1% less than the national average A1c (based on NHANES data).
All our patients have in common a desire to sieze the opportunity that JoslinCare offers—to become empowered, to control their diabetes and to live life to the fullest.
Joslin Works with your Primary Care Physician
Some patients ask how a referral to Joslin will affect their relationship with their primary care physician.
Primary care physicians, and other healthcare providers, refer their patients to Joslin because of the range of speciality services we provide. JoslinCare becomes part of your total healthcare plan.
Depending on your JoslinCare pathway, you may see a Joslin physician and other members of our team—nurse practitioners, nurse educators, dietitians, exercise physiologists, ophthalmologists, mental health professionals and others—as you need them, for occasional or regular diabetes care and follow-up.
Your Joslin team communicates with your healthcare provider, who continues to manage your overall healthcare and help with your diabetes between visits to Joslin.
